HOPE Connects

Diabetes Hope Foundation (DHF) is a grassroots not-for-profit organization founded by its current chair, Barbara Pasternak, in 1999 after two of her sons were diagnosed with diabetes. The foundation’s mission is to provide education and resources to help youth with diabetes transition to a healthier tomorrow.

The HOPE Connects Program supports youth aged 17-25 who are living with type 1 diabetes (T1D) and are transitioning into adult care and post-secondary education systems.

Four initiatives are provided through this program: 1:1 Peer Support; Collaborations with Campus Diabetes Clubs; Virtual Drop-in Spaces; and Online Youth Forum. Over three years, these initiatives will directly impact 555 post-secondary youth living with T1D, resulting in increased confidence and reduced isolation and diabetes distress.
